If you are certified but are no longer on the membership roll because you cancelled your membership or were removed from the roll, you may apply to renew your OTTIAQ membership. All you have to do is complete the application to resume membership form (French only) and the disciplinary decisions and court rulings form (French only) and email them along with your payment to the OTTIAQ secretary or mail them to the following address:

However, if your membership lapsed for more than five years, you must also send us details of any relevant work you have done since you left, along with a list of any courses taken and an updated résumé. After reviewing your application to resume membership, the OTTIAQ Board of Directors may require you to successfully complete an internship or a professional development course if deemed necessary to protect the public.
The Board of Directors will also require you to take the two-day course on ethics and professional standards of practice within six months of approval of your application if you have not already done so.
If your application is approved, you will be asked to complete the membership registration form and pay your membership dues for the current year.
